Practice Policies

Last updated: October 2026

Online only. All visits are by secure video, for teens (12+) and adults who are in Arizona at the time of the visit. Please join from a private, quiet place.

Your location: At the start of each visit, you'll be asked to confirm where you are, so help can be sent if needed. If you're outside Arizona that day, we'll reschedule.

If the video drops: Frances will call you at the phone number on file to finish the visit or set a new time.

Medication refills: Please ask at least 5 business days before you run out, through the patient portal or by calling or texting (480) 637-4174. Refills depend on keeping regular follow-up visits. Controlled medications are never refilled early.

Messages: Portal messages, texts and voicemails are answered within 48 hours. They are not for urgent needs.

After hours: Messages are not watched around the clock. In a crisis, call or text 988 or call 911.

Cancelling and missed visits

Cancel or reschedule at least 24 hours ahead. Then the time can go to someone else. Do it online from your confirmation, or call or text (480) 637-4174. There is no fee for cancelling.

Missed visit (no-show): $50. This applies if you miss your visit without cancelling first. It is not billed to insurance.

Payment, care and your rights

In-network: Aetna, Blue Cross Blue Shield of Arizona, Carelon, Cigna, Optum/UnitedHealthcare and UMR. Your coverage is checked before your first visit. Copays and deductibles are set by your plan.

Not accepted: Medicare (including Medicare Advantage) and AHCCCS (Arizona Medicaid). If you have either, please call before booking so we can help you find the right option.

Blue Cross Blue Shield of Arizona members: book through Headway.

Out-of-network? A superbill is available so you can ask your plan for reimbursement.

Self-pay: initial evaluation (60 minutes) $275. Follow-up visit (30 minutes) $150. Therapy session (50 minutes) $175. A sliding scale is available; please ask. Self-pay patients receive a Good Faith Estimate before their first visit.

Your right to a Good Faith Estimate: You have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost. Under the law, health care providers need to give patients who don't have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 1-800-985-3059.

Controlled medications (such as ADHD stimulants): These follow federal (DEA) and Arizona rules. Before prescribing, Frances checks Arizona's prescription monitoring program. Patients agree to use one prescriber and one pharmacy, keep regular visits, store medication safely, and not share it. Lost or stolen medication may not be replaced early. Telehealth prescribing follows current DEA rules, which can change.

Teens (12 to 17): A parent or guardian gives consent for care and joins the first visit. Teens also get some private time with Frances. What a teen shares stays private, except when safety is at risk or the law requires sharing.

Your records: You can ask for a copy of your records in writing through the patient portal or by email. We respond within 30 days.

Ending care: You may stop care at any time. If Frances needs to end care, she will tell you in writing, offer referrals, and, when it's safe, provide medication for a reasonable time while you transfer care.

Questions or scheduling: call or text (480) 637-4174, or email admin@breathepsychiatry.com.

Please don't email health details. We'll cover those privately at your visit or through the secure patient portal.

Privacy: Your health information is protected under HIPAA and Arizona law. See our Privacy Policy & HIPAA Notice (link at the bottom of every page).

Fair treatment: Breathe does not discriminate based on race, color, national origin, age, disability, sex, sexual orientation, gender identity or religion. If you need an interpreter or an accommodation, tell us and we'll help.

Concerns or complaints: Please contact us first so we can make it right. You can also contact the Arizona State Board of Nursing, or for privacy concerns the U.S. Department of Health and Human Services Office for Civil Rights. We will never treat you differently for raising a concern.

Emergencies: Breathe does not provide emergency or crisis services. In a crisis, call or text 988 (Suicide & Crisis Lifeline), call 911, or go to the nearest emergency room.
