Online care in Arizona: Visits are provided by secure video to patients who are physically located in Arizona at the time of the visit.

 

Medications: Treatment and medication decisions are made after an evaluation. Some medications, including controlled substances, follow federal and Arizona rules that may require additional steps.

 

Patient agreements come first: If you become a patient, your signed consent, privacy and financial forms and our Practice Policies govern your care. If anything here conflicts with them, those documents control.

 

Third-party services: Scheduling, messaging, video and billing are provided by other companies (such as IntakeQ, Headway, Alma and Spruce Health) under their own terms and privacy policies.

 

Links to other websites: Links to outside websites are provided for convenience. We don't control or endorse their content.
